Comber Physical Therapy and Fusion Chiropractic verifies insurance benefits as a courtesy to our patients; however, this is not a guarantee of payment. We make every effort to accurately explain insurance benefits, but please be aware that the information we provide is a direct reflection of information we receive from your insurance carrier. Payments will be determined when claims are received by your health insurance plan. If your insurance carrier processes claims differently than quoted, a bill or refund will be issued.

For those patients who do not have health insurance, are out of network, or those who have maxed out their benefits, we offer a private pay plan through CareCredit®. Please note that a quote of benefits and/or authorization does not guarantee payment on services rendered. Payment of benefits is subject to all terms, conditions, limitations and exclusions of the member’s contract at time of service.

Your health insurance company will only pay for services that it determines to be “reasonable and necessary.” Every effort will be made by this office to have all services and procedures preauthorized by your health insurance company. If your health insurance company determines that a particular service is not reasonable and necessary, or that a particular service is not covered under the plan, your insurer may deny payment to patient’s responsibility.
